Community Liaison
Schedule:
Monday–Friday, 9:00 AM – 5:30 PM (flexibility for evenings as needed)
Boys & Girls Haven is seeking a Community Liaison to support both Pre-Independent Living (Pre-IL) and Independent Living (IL) programming. This role includes administrative coordination, client support, and case management responsibilities to ensure continuity of care and successful transitions as youth move toward independence.
What You’ll Do:
Support administrative and program needs within Pre-IL services
Provide case management for IL clients, including coordination of medical, transportation, educational, and employment needs
Assist clients with enrollment in school, vocational training, and trade programs
Maintain communication with schools, training programs, and service providers
Connect clients to community resources such as SNAP, food banks, identification services, transportation training, and volunteer opportunities
Provide CSA sessions
Assist with Pre-IL intake, discharge, and client needs
Schedule and document Pre-IL and IL treatment team meetings
Support client engagement activities (holidays, birthdays, donations, etc.)
Assist with apartment turnover and preparation for new IL clients
Transport clients to appointments and advocate on their behalf as needed
What We’re Looking For:
Bachelor’s degree in a human services-related field
Targeted Case Manager (TCM) preferred
Experience working with youth, particularly in foster care, residential, or similar settings
Strong organizational, communication, and case management skills
Ability to manage multiple responsibilities and work independently
